systems which track and process Commission claim information, as well as to support the
users of these systems.

What are the costs for reports?
Standard reports cost $25 to $50, depending on the report. Customized reports start at
$50 and go up from that figure, depending on the amount of time involved in creating the
report. We reserve the right to decline to provide custom reports if the time investment
is too great.

How do I obtain a copy of the Annual Report?
To obtain a copy of the Annual Report, you may call or write the:

Please state the report year you are requesting. Quantities may be limited for some years.
There is no charge for a copy of the Annual Report.
